The following error message may indicates a SAN issue, this is not an adapter hardware issue.

What This Means and what to do What to do
0x0000 001C A device in the SAN has refused a connection (login). This is usually a SAN configuration issue.
0x0000 001E A device in the SAN sent a response and AIX does not have an active login with the device. This is a SAN device issue
0x0000 0038 These messages are logged when the command structure that the device driver is attempting to use is in use.
This indicates the a device in the SAN is not responding to command.
  • If connection type is 2 (fabric), check the connection status with the switch
  • If connection type is point-to-point, check with the end device determine why the device is not responding.
0x0000 0039
0x0000 003E The Adapter can not switch to FC-AL, but that appears to be the way the SAN is configured.
Either re-configure the SAN or change the Smitty FC-AL setting.
0x0000 0048 A device in the SAN sent a logout (LOGO) to the adapter and fields in the frame were notfilled in by the device. The problem is with the device that sent the logout,
0x0000 0049 A logout by a device in the SAN used a port name that does not match the entry in the login table. The AIX port_name is stale, contact AIX support.
0x0000 004F The SAN has rejected the cmd due to a bad source ID. This may be a transient fabric issue, if it persists, contact AIX support or SAN central..
0x0000 0053 An IOCB failed with an LS_RJT. The failed cmd is included. Action depends on failed reason code.
0x0000 005B The device driver has received a (PDISC) command from a device that is not registered (no login) with the AIX server. his may indicate a transient SAN issue, if it persists, contact AIX support or SAN central..
0x0000 005D An unexpected command has been sent to AIX from the SAN. This may be a transient SAN issue, if it persists, contact AIX support or SAN central
0x0000 0090 This message is logged when a FAN (Fabric Address notification) has been received and the topology is not AL. . This is an informatonal message, if it persists, contact AIX support or SAN central
